3. Customer to receive text notification from EO-Executive Optical.
4. To complete the entry customers must register online via the EO microsite with the following details (First Name, Middle Initials, Last Name, Registered Contact Number, Complete Address, Transaction Number, Date of Purchase, Total Purchase Amount)
5. Deadline for submission of entries is on April 15, 2023, 11:59PM.
6. Determination of winners will be held on (April 16, 2023/2:00 PM/One Oculus Center Makati City) in the presence of a DTI representative.
7. Major Prize:

* Major prize is non-transferable

8. Announcement of winners is on April 18, 2023, at 10:00 AM via EO social media accounts.
9. Winners will be notified via registered mail from EO-Executive Optical.
10. A participant can only win once.
11. Minor Prize Winners may claim his/her prize at their preferred EO branch. 12. Major Prize Winner must claim his/her prize at Executive Optical Head Office located at One Oculus Center, Chino Roces, Makati City.
13. Winners must present the following upon claiming of prize:
- Two (2) government-valid IDs (Passport, voter’s ID, Driver’s License, etc.)
- Notification text message and email from EO-Executive Optical
- Notification Letter from EO-Executive Optical via Registered Mail
14. In case of the absence of a winner, an authorized representative must present the following upon claiming on behalf of the winner:
- Duly notarized authorization letter of winner with signature
- Photocopy of two (2) valid IDs of the winner
- Photocopy of two (2) valid IDs of representative
- Screenshot of text notification sent by Executive Optical (Email & Text)
- Notification Letter from EO-Executive Optical via Registered Mail
15. Winners must claim their prizes 60 days from receipt of notification. Otherwise, prizes will be forfeited in favor of Executive Optical with prior DTI approval.
16. All employees of EO-Executive Optical, its agency, and their relatives up to the second degree of consanguinity or affinity are excluded from joining the EO Summer Lovin’ Giveaway Promo.
